Scars come in a wide variety of types and it is safe to say that any injury causing an opening through the skin will produce a scar. The mechanism of injury is an important part of how a scar will heal. Tearing the skin open with a hammer will tend to produce a more unpredictable healing result than making a nice incision with a scalpel. If infection occurs that may alter the healing process and cause a laceration or incision to open and drain and this may cause widening of the scar.

Heredity is a definite factor is how people heal scars. Scotch, Irish, English heredity tends to produce the best scars. Hispanics, African Americans, and those from Mediterranean countries tend to not heal as well.

Scar revision can be performed as an attempt to improve a scar by removing the old scar or performing a Z-plasty. Thick, raised, red scars are referred to as hypertrophic scars.

They may improve over 6-9 months from the time of the injury or may need to be treated with Triamcinalone (Kenalog) injections into the scar to help soften and flatten the scar.

A keloid is an over-growth of scar tissue and the classic example is walnut-sized scar from piercing an earlobe. The keloid has extended beyond the site of the original injury. Keloids can be treated with Triamcinalone injections, scar revision and removal of the entire keloid, electron beam radiation after excision, and injection of both Triamcinalone and an 5-Flurouracil together after excision. When it comes to difficult scars, nothing is guaranteed to work and sometimes multiple different treatments are necessary.
